It is likely that I will get my MM2H visa end of 2017. I am contemplating on staying in Malacca or Langkawi. I have visited KL and I don't like big cities. Penang is OK but I prefer somewhere slower. East Malaysia is out of the question as MM2H does not allow one to stay there. So, I am left with only Malacca or Langkawi. Malacca at least have good medical facilities whereas Langkawi has none. Met up with a few foreigners staying there and they will either go to Alor Setar or Penang for medical treatment. That is the sticking point. The best point for Langkawi is cheap booze and nice and quiet beaches. Any comments anyone?

Go for Penang or KL wold be my advice. Both are easy to travel from with connections to SG, BKK, JKT. Best area in Penang for foreigners is north of Georgetown between Tanjung Tokong and Batu Ferringhi. Housing is a bit more expensive in these areas but overal cost of living still cheaper than KL.
Penang itself has a good infrastructure and just the right size. If safety is a concern then Penang is definitely better than KL. Over all I just find Penang more relaxing and unless you want to do business then there really is no reason to move to KL... then rather SG or BKK.

I am in the MM2H business and I am based in Penang. However, I have helped many people apply for the visa and most of them stay in Penang. There are many younger ones staying in KL but of course KL has all the issues faced by big cities - higher pollution, congestion, crime, cost of living, etc. However, you can get great nightlife and a lot of social activities. Penang on the other hand is laidback and nightlife, is well, rather pale in comparison to KL. It does have great medical facilities that many retirees are looking at as well. Langkawi not a good choice because it is an Island, not really convenient for normal living but good for vacation.
Either Penang and Malacca is good.
Penang quite well developed but too far away from Kuala Lumpur and international airport (4 hours drive). Higher property price. 
My hometown Malacca is not as developed as Penang but just within 2 hours drive to Kuala Lumpur, slower living tempo and lower property price.

Malacca is one of the places to stay in Malaysia. How is the medical facilities stack up against Penang ? I know Langkawi has poor medical facilities.

I'm not sure with Penang. Malacca medical facilities quite good, not only those in hospital but also some small private clinic working well.
Few of my friends from other state recommended to seek for doctor in Malacca.
Distance from Malacca to Kuala Lumpur is nearer compare with Penang. Major or serious incident may need support from Kuala Lumpur.

Well..... it depends on where the foreigners want to stay. For better or worse, many choose Penang...
